Accident Coverage
Accidents are a part of life and can happen more frequently than we expect. With Dearborn Accident coverage, you are purchasing a plan that will reimburse you with level dollar amounts depending on the type of accident. The extra money can be used in any manner you wish and can cover costs outside of your group medical plan. The Accident Plan also includes an annual Wellness benefit of $50.00 per person enrolled. Critical Illness Coverage
No one plans for a critical illness but having essential supplemental insurance coverage during a major medical event can bring peace of mind and financial support when it is needed most. Critical Illness Insurance protects you in the event of a future major illness diagnosis. The plan supplements existing health insurance coverages with extra funds that help to cover increase expenses, medical or otherwise, that face when you suffer a critical illness. Proceeds from any approved claim may be used however you wish. Rate are determined by your age bracket and desired amount of coverage. There is a guaranteed issue amount of up to $20,000 for anyone enrolling withing the first 31 days of active employment. Requests for coverage at any other time, including Open Enrollment, is not guaranteed and you will be required to complete an Evidence of Insurability form (statement of health) for BCBS/Dearborn National to review. Short-Term Disability
Short-Term Disability coverage is a convenient, economical way of securing an income while out of work from an unexpected injury or illness. Conditions related to pregnancy and/or childbirth that result in a person's inability to perform their own occupation may also be covered. Voluntary Group STD is a guaranteed issue coverage, which means it does not require a health questionnaire to complete. You may choose a weekly benefit amount from $100 to $1,500 in $50 increments, not to exceed 60% of weekly earnings*. Rates are based on your age bracket and desired amount of coverage. *Weekly Earnings means your weekly rate of earnings from your employer in effect immediately prior to the date disability begins. It includes total income before taxes including deduction made for pre-tax contributions to a qualified deferred compensation plan, Section 125 plan, or flexible spending account. It does not include bonuses, overtime pay, or any other extra compensation.
Long-Term Disability
Long-Term Disability insurance allows you to financially protect yourself and your family by providing a convenient and economical way of securing income while out of work for an extended period from an unexpected accidental injury or illness. Voluntary Group LTD offers benefits for total or partial disability and covers up to 60% of your basic monthly earnings*. Rates are based on your age bracket and monthly salary, not to exceed $10,000. Enrollment is guaranteed only within your first 31 days of active employment. Requests for coverage at any other time, including Open Enrollment, is not guaranteed and you will be required to complete an Evidence of Insurability form (statement of health) for BCBS/Dearborn National to review. Voluntary Life/Accidental Death & Dismemberment Insurance
The death of a family member can mean not only dealing with the loss of a loved one, but the loss of financial security as well. With Voluntary Group Term Life, an employee can achieve peace of mind by giving their family the financial security they can depend on. Group AD&D is an additional death benefit that pays in the event a covered member dies or is dismembered in a covered accident and carries the same coverage level Voluntary Term Life. Rates are based on your age bracket and desired amount of coverage. Enrollment is guaranteed only with in your first 31 days of active employment with a guaranteed issue amount of up to $225,000. Additional coverage may be purchased for a spouse (up to $50,000, not to exceed 50% of employee coverage) or dependent child (up to $10,000).
